Your website should have a category called 'Accessibility' where users can find all the information that pwd need to access the app. There should be a page where users can submit the accessibility issues they are experiencing.

(And, BTW, This form says: "You must be signed in to add attachments." I am signed in and it has not allowed me to add an attachment.

Keep in mind that accessibility issues can be reported in the relevant board of the Community page, or directly to our Support team here. We'll also be happy to receive all your feedback regarding accessibility and pass it on to the right folks. 

Additionally, one of our Idea Exchange Guidelines is that you should only post one change request per idea, so make sure to keep these guidelines in mind if you'd like to suggest something in the future.
