Offline playlists on my iPhone 4 desynced and set back to 'online' mode

My wife and I both share a Premium account, and we don't have more than 3 devices with offline playlists. Just yesterday both of our iPhones desynced our offline playlists (deleting all the music from our phone drives), and set them back as 'online' playlists. 

Does this happen with you Sptify vets? I am still within the 30 day free trial but honestly if this keeps happening then Premium isn't worth the money.

Do you get any messages come up when they're de-synced? If it's an issue of having more than 3 devices, it should say something like 'offline access for this device has been withdrawn' or something similar.

If not, how much free space do you both have on your iPhones?

I found out that not only were the offline playlists on our phones de-synced, but also on my computer.

Is that standard procedure to de-sync every playlists on all devices used with the account when more than 3 devices are setup to have offline playlists?
