Media volume constantly resets to middle of slider

When I adjust the volume in the Android app, it will revert back to the middle of the slider each new track or advertisement. The result is only being able to listen at the desired volume level for a couple minutes at a time, and then having to readjust again.

When I try to lower the volume sometimes the slider will immediately go up again, like I'm fighting over the volume level with the app.

The media volume slider sensitivity is different in Spotify as well - about 50% of the middle section sounds the same volume level.

It really is annoying if I have a phone call and could be dangerous when I'm driving.

I've tried enabling/disabling the equalizer, and turning on/off track volume normalization, without any luck. I've searched for fixes, but haven't found anything like I'm describing.
