Just to confirm, is this only happening when you're listening to the Radio playlist on your phone or this happens on other devices (such as a PC) as well?
On another note, we suggest that you log out everywhere by following these specific steps.
Lastly, would you mind sending us a screenshot of the Queue where we can see the random song playing (outside of the Radio playlist)? We'll take a closer look. You can attach it to your next response by using the Insert image option in the post editor. Make sure not to send any private info.

I have this issue as well if I’m understanding your post correctly. When listening to song radio, while I’m listening the order of the radio will change. Suddenly songs will be replayed and if I look at the radio playlist, the song being replayed will have moved from where it was to further down the playlist with a new song that was not previously in the playlist replacing it further up. This will happen continuously with the same songs. So a song will move down the playlist to be the next song, I will skip it and a few songs later it will have moved once again to be the next song. Usually there are a few songs in the playlist that do this. I’ve had this issue on my iPhone and on the web player.

It's important to first isolate the described experience to the usage of only one device. If you're using Connect and more than one device - stop for a bit, but make sure to clear the play queue on all the devices you've been using. Here is how to do it. Some devices may not have this ability, but if they do - clear the queue.
Now try interacting with the radio on only one device. Are you able to reproduce the experience? If so, could you upload a short screen recording of what is exactly going on, so we get a better understanding? You can upload it to YouTube or Google Drive and post a link here.

I think I am experiencing the same issue as the author of this post is describing for his BMW G20.
I am also using the native Spotify App of the BMW iDrive 7 system and I have issues with weird behavior of auto-generated playlists, like the following:
(This issue does not apply to static playlists, like those which I create by myself)
The issue was as follows: I would select a playlist from one of the above mentioned types and after a while (sometimes even the very next song), it would not play correctly. Instead, it would play a completely different song from the list (shuffle mode disabled).
Even when explicitly selecting a song from the list, it would play a different song at a different position in the same list. Selecting the desired song again, would result in the exact same, wrong song being played.
Since for me this issue only occurs with auto-generated lists/mixes, which are periodically updated, it feels like the list cached in the BMW Spotify App is out of sync with the actual state of the list on the Spotify servers.
Meaning, in the BMW App, I would like to play a particular song, which is in shown to be in position X of the list, however, on the Spotify server, the list has already been updated but not communicated with the car App, so that there is a different song in position X, resulting in the wrong song being played.
So far, I have only ever experienced this issue in the native Spotify App for BMW iDrive 7. However, a friend of mine told me he experienced a similar issue with the Spotify integration for Discord bots, where random generated playlists suddenly seem to be out of sync with the Spotify servers, indicated that it may not be an issue exclusive to BMW.
I hope my inputs are helpful in finding a solution.

First up, could you try checking if the software of the iDrive 7 is up to date?
If so, we know it's not ideal, but it's worth resetting the vehicle's data and if it's a cache issue, that could help.
If the issue persists, would you mind sending us a short video of the issue happening so we can take a closer look? You can attach it to your next response by using the Insert Video option in the post editor. You can also upload it to Google Drive and share the link with us (make sure the video has the permissions for anyone to see it). You can do it whenever you have time, just avoid avoid interacting with your device while the car is in motion.

Please excuse the very long delay - I actually recorded the video almost a month ago, but I noticed that it contained some sensitive information, which I had to blur out, and I only today found the time and opportunity to edit and upload the video.

As you can see in the video, the latest available software version for my particular car is installed, which is 07/2023.23 (it is to this day the latest version).
Resetting the car to factory defaults is something I would like to avoid, due to the risk of losing some important data and settings.
Instead, I tried a forced reboot of the car's headunit as well as logging out of the integrated Spotify app and logging back in - however, none of these actions solved the issue.
I hope my video clearly shows the problematic behavior and helps you with finding a solution for the issue.

Upon further investigation, we've been told by our tech team that they were already aware of this and they've started looking into this and working on a fix, but sadly there is no timeline we can give at the moment on when or if this will be addressed.
