This problem has come up before with somebody saying they disabled hardware acceleration in settings – an option which doesn't exist (anymore).
So what can be done when the radio doesn't load?

Have you tried another network connection? Could you try creating another user account and installing Spotify there?

I've accepted your answer although the fix in my case was a bit different. I did have a one entry in my hosts file which redirected some spotify domains into the abyss.
It was installed as part of a ad- and malware blocking hosts file from GitHub (probably this one)
I removed everything with spotify in it (in my case 0.0.0.0 spclient.wg.spotify.com) and the radio tab now loads.
As a side effect, the previously empty Recently Played section is now also populated.

I know it sounds simple, but it's often overlooked. Could you try restarting your device? If that doesn't help, try a quick clean reinstall of the app. Just follow these steps.
If you want to disable Hardware Acceleration, just click on the spotify Menu in your Menu Bar next to the Apple Menu, just find Hardware Acceleration and next click on it to disable then confirm to restart Spotify.
If you're still having trouble, could you let me know the exact version of Spotify and the exact version of the operating system you're running?
