My wife and I live together and she has been removed from family pack. Why that?

Probably it's because your wife forgot to click on the link that was sent to here per email, to confirm that you live together. 

What I would suggest to do here is to cancel your existing subscription and immediately reapply.

Hi, thanks for replay but muito wife accept the confirmativo a few days ago. And today that happens. I sent New invitation but when finish the name, adress, city, etc. Appears 3- something wrong, teu again.

And now i dont know If she is blocked in my account...

Having no spotify premium isn't cool! Usually these kind of things happen, because of your web browsers cache. So try doing the following things:
Try Opening the link in a private/incognito tab.
If that doesn't work, change browser and clear both browsers cache.
Lastly if that doesn't work either, try opening the link on another device, that should do the trick!

Thanks for the help, but didn't work.

I don't know how I can talk to support from email and solved that.
