How on earth do I accept a premium for family invitation?

I’m trying to add my wife to our premium for family account and every time we try to enter the confirmation information it says “Oops something went wrong”.  I’m assuming it doesn’t like what’s being entered for street name and number, but I have no way of knowing as I can’t look that up in my account.  I’ve tried entering that info 30 different ways and I can’t get a match.  How do you do this?
