tried to fill out the "SheerID verification form", but the site isn't open

Currently i am using "premium individual" and since i wanted to change it into "premium student", so I tried to fill out the "SheerID verification form" and vertify that im student, but the site is not opened repeatly. So can you give me the other proper link or other ways. I have already tried it also in incognito window.
