Individual Plan/Return to Spotify Premium Campaign issue

Currently Free, now trying to return to Premium.

Was Premium early adopter for several years prior.

 

My Question or Issue

Had issue with credit card fraud a few months back which was solved. Now I cannot upgrade back to Premium. Have attempted via current Return to Premium Campaign several times with different payment methods and cannot successfully subscribe.

 

Have spoken to Bank and their end all seems fine, although the request from Spotify to process payment doesn't even appear to them, so I presume some issue with campaign vs. payment.

Payment Attempted via webmail, anonymous Chrome window and normal window.
