For whatever reason, the problem appears to have fixed itself. The only thing I've done is change the streaming quality to very high and auto adjust to off, but even that didn't help right away. It might have been patched. All other Spotify and speaker settings are at their defaults. Best of luck to anyone with this problem and thanks for the help rendered!
The problem popped up again a couple of days ago. I was able to fix it this time by going to system -> sound -> select your speakers near the top under output -> spatial sounds under "Advanced" near the bottom -> select Windows sonic for headphones

A complete re-install eliminates likely any cache issues and also could you ensure you're running the latest version of Spotify
If the issue persists, try installing the app from a different source to see if that does the trick: if you downloaded Spotify from our website, try it from the Microsoft Store instead and vice versa.
This did not fix the problem unfortunately. I've discovered that tinkering with any setting on my Realtek speakers (control panel->sound) returns the audio to what seems like normal but changing the slider and reopening spotify seems to break it again.

You can also try resetting your network adapter (in Windows) by right clicking the wifi symbol on the right side of your toolbar. Then click diagnose and select the option to reset network adapter. The problem came back for a short while and this seemed to fix it.
