The large Spotify widget updates the latest playlist/album/artist that is playing or has been played but the other 4 icons do not change. Has anyone else experienced this and do they know a solution?

Just to be on the same page, have you tried to perform a clean reinstall of the app on your device? If not, would you mind following the steps mentioned in this article to see if it makes any difference?
If the issue persists, it would also be great if you could have someone log in with their account on your device - such as a friend or a family member to see if the behavior persists.
Lastly, include in your next response the Spotify version you're currently running to continue investigating the case.
We'll be on the lookout.
I have the same problem, and i tried nearly every solution on the internet. Doesnt work.

We've reported this to our tech team who'll investigate further. In the meantime, make sure you're always running the latest version of Spotify so you don't miss out on any updates.

Same here. Tried everything, still blank. My Spotify is running on the latest version too.

I had this too. Newly set up iPhone, installed Spotify app, added the large widget to my Home Screen and signed into Spotify. Played some music from 1 playlist but bottom row stayed “empty”. I tried closing the app and deleted and re-added the widget at the beginning. Then I just let it be. It seems to me that listening to a 4th different playlist/album/podcast item just now fixed it. At least these 4 items now appeared in the bottom row. It’s been ~8 hours.
