Fix the Gift Card Delivery Date Input Box

Problem: the date format is YEAR-MONTH-DAY, which is backwards from every other form in existence.  But, most importantly, there are no instructions as to this format--I had to search online to find it after failing on every other format I could think of.  ADD A SIMPLE EXPLANATION HOW TO ENTER THE DATE ON THE PAGE.  Embarassing web design.
