We've tested this one our end, however, we couldn't reproduce the issue. We therefore recommend reinstalling the app and removing Google from the app list on your account page to see if that makes a difference on your device.

I'm having the exact same issue. I'm using the stock google clock app https://play.google.com/store/apps/details?id=com.google.android.deskclock
It's not linked in my account.
I use a spotify playlist as alarm tone. If that playlist has smart shuffle active the alarm uses the media volume. If it doesn't have smart shuffle active it uses the alarm volume as expected. This makes using spotify as an alarm incredibly unreliable. It must use the alarm volume even with smart shuffle to be reliable.

Could you and @sbryson check the app permissions, as well as Google Alarm's settings if you could toggle any of them to change which audio output the app is using?

There is nothing in the clock/alarm options that let's me choose which volume to use. Just a slider for the alarm volume. I'm just choosing a playlist (one created by me in my case). I double checked that the clock app has all permissions including overriding dnd before posting.
The switch between volumes happens when I turn smart shuffle on in the spotify app. Without smart shuffle (regular order or shuffle) it uses alarm volume as expected and with smart shuffle it uses media volume. I can then even adjust the sound of the alarm while it's ringing by changing the media volume slider. The problem is my media volume before sleep is usually silent, so my alarm won't work if smart shuffle is turned on.

This reddit thread reported the same problem and also figured out the connection to smart shuffle. There is definitely multiple users affected by this.
I'm on a Samsung Galaxy S23. The reports on Reddit are from a Google Pixel.

In order for the relevant team to be able to do a further investigation, would you mind sending us your Android version, as well as the exact Spotify version?

I added screenshots of the android software info and spotidy app info. I retested the issue also one more time and it's still there and reproducible.
