I get an error message saying I have already created an account with this user name when I try to log in.
Thanks but it doesn't help. I have uninstalled, turned off, reinstalled. Still the same error message. The app is not working

That doesn't sound good you're getting an error message when trying to login. Let's try to get you sorted out and back into your Spotify Account on your iPad.
You may be getting the error about already having an account created with that username if you're attempting to create a new account instead of choosing the Log In option on the Spotify Mobile app for iPad. Even I've done it before by mistake, so just be sure you've chosen the Log In option and not creating a new account.
It's great that you've tried the troubleshooting guide as suggested by @ErikG - that's where I'd also start for an issue like this. I would recommend doing another uninstall of the Spotify App, but instead of just restarting your iPad afterward, I recommend doing a soft-reset by holding down the Home and Power buttons on your iPad for 10 seconds, or until the Apple logo shows up.
Note: a soft-reset should NOT cause any data loss on your iPad - it merely crashes iOS, giving it a chance to boot up from a completely cold (off) state instead of restarting.
Then reinstall the Spotify Mobile app from the App Store and let me know what happens. You could even post a picture (taken with a cell phone or other camera) in your reply so we can see exactly what screen you're on when this error ocurrs.
Did the soft reset. No change.
I select login (not create new) enter my email and password. It thinks for a couple of seconds and then a pop up window appears saying login failed you've already created an account with this email address. If I use my username instead then the error message is login failed unable to login, either username or password is incorrect. Have you signed up for a Spotify account?
Thanks for your patience and making sure you're logging in and not signing up for a new account. It really helps narrow down further suggestions to try.
Just to test things out, can you attempt to login on the Spotify Website to check your account? https://accounts.spotify.com/
I know this might seem silly if you're having problems logging in on the iPad app, but I'm asking to make sure your account username/email and password are actually working correctly. If you can login on the website, it'll be good to let Spotify Support know this.
Let me know if you can login on the website and we'll take it from there. If it continues to give you an incorrect password error, go ahead and try resetting your password through the website, just for giggles. You never know with computers - they can do strange things sometimes.
