Right. I think the SD card is the culprit and I also think that it's correupt and faulty.
You should contact Sandisk using the form here and provide them with all the detail of things you've tried. You could send them a link to this thread so they can see a history of what we have tried. Hopefully it should be covered by standard warranty and they'll do an RMA for you.
